1. Download the Major/Minor Declaration Form
2. Fill out required sections
3. Bring form to the AAAS Department (1046 Humanities Building)
4. Ask to have it signed by the Department Chair (Dr. S.N. Sridhar) or the Assistant to the Chair (Darlene Prowse)
5. Bring completed form to the Registrar (Administration Building) to have it submitted into your record
