Online Forms
The online form to file for graduation is no longer on this website. If you expect to graduate this term, you need to log on to SOLAR to submit a graduation clearance form.

If you have applied for graduation but now need to change your graduation term, you should print and submit the Change of Graduation Date form (PDF file).

Forms To Be Printed
To view and print these "paper" forms, you will need Adobe Acrobat Reader (or some other PDF viewer). Acrobat Reader comes pre-packaged with newer web browsers, but if you do not have it, you can download it for free.

• Academic Dishonesty Policies and Procedures
• Academic Petition (for adds after the add deadline and withdrawals after the withdrawal deadline)
• Transfer Request Form

GRADUATION CLEARANCE FORMS
Graduation clearance forms are now submitted via SOLAR. However, if you have applied for graduation but now need to change your graduation term, you should print and submit the following:

Change of Graduation Date (This form is required for students who have ALREADY APPLIED for graduation, but now need to change their graduation date.)

If you cannot print out the form you need, email SPD and one will be mailed to you.
